Forms for Special Circumstances
These forms are needed when a family will be hosting two AFS students:
2018 Student and Natural Family Consent (prior to program)
2018 Student Consent Only (for a move once a student is on program)
2018 Natural Family Consent Only (for a move once a student is on program)
If a family will be hosting an AFS student and a student through another organization, the above forms are needed for the AFS student only, in addition to the following form (consent from the other organization's student and natural family are NOT required):
Use this form for Single Parent Approval if the student is already in the U.S. and will be moving to a Single Parent home. AFS staff will seek approval from the natural family or when the student has not yet arrived in the U.S.:
Same Sex Host Parents
Use this consent form if the student is already in the U.S. and will be moving to a home with same sex host parents. AFS staff will seek approval from the natural family or when the student has not yet arrived in the U.S.:
