Forms for Special Circumstances
To learn more about hosting situations that require special consent and to find the process to use for identifying students open to these placement types, visit Get Consent for Placements Requiring Supplemental Paperwork (formerly known as “Non-Traditional” Approval). Double Placement
These forms are needed when a family will be hosting two AFS students (Note: AFS-USA now collects these consents through an e-signature process):
- Host Family Consent Form
- Student and Natural Family Consent (prior to program)
- Student Consent Only (for a move once a student is on program)
- Natural Family Consent Only (for a move once a student is on program)
If a family will be hosting an AFS student and a student through another organization, the above forms are needed for the AFS student only, in addition to the following form (consent from the other organization's student and natural family are NOT required):
One Member Household (formerly “Single Adult”/“Single Parent” Placements)
Use this form for One-Member Household Approval if the student is already in the U.S. and will be moving to a One-Member home. (Note: AFS-USA now collects these consents through an e-signature process).:
Event Family and Short Program (VS) Families
You can use this form to complete an interview for an Event Family or a Short Program (VS) Family:
